SPORTS PHYSICALS INFORMATION

I just wanted to help provide some low-cost options for obtaining a sports physical. If your child is interest in playing school sports, please remember, your child MUST have a completed sports physical uploaded to their registration portal in order to be eligible for tryouts and practices for their sport of choice.
You can look at your local CVS locations that have a MinuteClinic ($35 - $70) or any Urgent Care Clinic ($20 - $25). They also do sports physicals. Local Urgent Care Centers are Walk-In Friendly.
Please also see the flyer below regarding back-to-school heath clinics in Pinellas County. One of the services provided are SPORTS PHYSICALS. As we prepare for the coming school year, please get ahead of your child sports registration by getting the sports physical completed early. Enrollment for the 2025 - 2026 school sports registration will open June 1st. Now is the time to make your appointments. If your child is in 5th through 8th grade and want to play sports, this is a great AND FREE resource. The updated sports physical form is below.

PSL REGISTRATION INFORMATION
Below, you will find all the links to the necessary paperwork for the Plato Sports League. A sports physical will be required for your child to participate. The form is also below so you can print it off and give to your child's doctor. Please note the following:
- In order for your child to tryout this year, ALL paperwork, including a sports physical, MUST be filled out.
- If all paperwork is not complete, your child will not be permitted to tryout for a sport.
- The registration portal will close (2) days prior to any tryout to allow administration time to perform an academic and conduct check on those students.
**PASCO COUNTY**
Pasco D.O.H. does not provide sports physical. Please contact your local pediatrician, CVS MinuteClinic, or Urgent Care Centers. Urgent Care Centers are cost-effective and are walk-in friendly.
Starkey Ranch ER will do sports physicals for FREE, as long as you bring the forms in with you.
**HILLSBOROUGH COUNTY**
Hillsborough D.O.H. does not provide sports physical. Please contact your local pediatrician, CVS MinuteClinic, or Urgent Care Centers. Urgent Care Centers are cost-effective and are walk-in friendly.
