For the 2025-2026 school year, incoming 7th and 8th grade students who want to participate in athletics must have the following steps completed before the first tryout or practice. Students will not be allowed to participate until they have ALL steps completed and all fees paid.
Forms that must be turned in to the office (also available on the MTMS Website under the “Athletics” tab):
- KSHSAA Physical – physical must be completed after May 1, 2025, by a MD, DO, DC, PA-C or APRN (please ensure you, your healthcare provider, and your student have signed and dated all required sections. Incomplete forms cannot be accepted).
- Please ensure you are submitting the most up-to-date KSHSAA Physical form (Revised 3/2024) with all five (5) numbered pages. Please double-check that any form provided to you by your healthcare provider is the most recent version. Previous years’ forms cannot be accepted. You may submit your completed physical in person at our main office until June 6th, through the school drop box outside our front doors, via our online Form Turn In (found under the “Athletics” tab at the bottom of the page) (please submit a clearly scanned copy), or in-person when school offices reopen on July 23, 2025.
Forms to be completed online through Skyward Access (Due to Skyward updates, these forms will not be available until enrollment opens in July). (Both parent and student will have to sign-in to their respective Skyward accounts to complete):
- Acknowledgement of Risk
- Athlete Code of Conduct
- Concussion Form
- Emergency Medical Authorization (EMA) Form (Parent completes)
- Transportation Authorization (Parent completes)
Fees to be paid:
- Catastrophic Insurance Fee (This small fee is required regardless of student's personal health insurance availability)
- Activity Participation Fee
Deadline for fall athletes to complete the athletic registration process is August 8, 2025 (to allow time for processing). Fall athletics include Volleyball, Cross Country, and Football.
