If you did not fill out Dr. Galdston's survey identifying which learning model you will be choosing for April 26 through the end of the year, you must fill out this survey for our planning purposes by Tuesday, March 23 at 3pm.

If you do Not fill out either form, your student's learning model will default to the following:

• Current hybrid students will default to full in person learning, 5 days a week
• Current remote students will remain remote

If you filled out the re-entry form from Dr. Galdston, there is no action necessary.
