This could be a student ID, bus pass, dated letter from a principal or social worker, class schedule or enrollment form.

The PTA Clothing Center in partnership with Replenish KC is giving away new socks, underwear and backpacks full of school supplies on August 8, 2023 from 6:00 - 8:00 PM at the Clothing Center at 8009 N Atkins Place. See flyer below.

Donations are welcome at the Clothing Center. Please make sure your donations are clean, in good condition and bagged in kitchen-sized bags that the volunteers can lift.

Health Room News

ATTENTION INCOMING SENIORS: For the 2023-2024 school year, your senior student must receive the second dose of their meningococcal vaccine. Please make sure your child receives meningococcal A. Most students commonly get MenQuadfi, Menveo, or Menactra. Second doses (booster) are due once a student has turned 16 years of age. Any student who plans to participate in fall sports must submit an updated immunization record prior to the start of fall try-outs and practice. Otherwise, students must submit an updated immunization record prior to the first day of school in order to attend classes.

Missouri State Law requires that evidence of immunizations be a statement, certificate or record from a physician or other recognized immunization record. The statement must include the type of immunization given and the date (month, day and year) of administration. Students may receive the necessary vaccines through their personal physician's office, various Minute Clinics, or the Kansas City Health Department. Let us know if you need a copy of your student's immunizations. Please call us if you have any questions or concerns at 816-359-4114 or email Ashley Hendrickson BSN, RN at HendricksonA@parkhill.k12.mo.us.

NOTE: According to state law, your child will not be able to start school unless we have proof that your child has the immunizations required by law. If you have not submitted a current immunization record, your student's technology access was terminated on July 14. Technology access will be turned back on 24 hours after immunizations are received and verified by our nurse.

Park Hill Website - Health Room

Please visit our website at https://phhs.parkhill.k12.mo.us/our-people/nurses-clinic for forms to have medication in the health room. If your student needs to carry an epi pen or inhaler on them during the school day or keep it in the health room, please check the website for forms regarding epi pens and asthma. If your student is diabetic, please see the self-care diabetes form.

  • Student's Grade Level
  • Reason for the absence or late arrival
