Weekly Update 9/12

Dear Oak Brook parents and families,

The Visiting Nurse Association will be hosting a student Influenza Vaccine Clinic at Oak Brook On October 10th from 12:45-2:30pm.

This is an optional clinic for students to participate. Please see the attached copies of the

Vaccine Information Statement, Consent forms, and accepted Insurances.

If you would like your child or children to participate, read the consent form then complete,

initial, and sign one form per child, and send it to the Oak Brook school nurse by October 1,

2025. Please attach a copy of your child’s insurance card with the completed consent form.
