🏀Winter Sports: Boys Basketball, GirlsBasketball, Cheerleading

Click here to indicicate which winter sports your student will be trying out for. This form closes 10/30.

PLEASE NOTE: Students have their own form in the Google Classroom. If they are on either list, and all forms are completed by 10/30, they are able to try out.

All forms and paperwork must be completed and submitted ON GENESIS by October 30th.

• Log in to your Genesis Parent Portal
• Click on Forms (on the left side)
• Click Winter LMS Sports and follow directions

• Sports Physical must be printed, completed by a doctor and uploaded into Genesis. The last page MUST be signed by the physician. Physicals are valid for 364 days.

• Health History Update form must be completed by a parent and uploaded into Genesis. This is required for all student-athletes, every season.

• Opiod Misuse, Concussion Fact Sheet, Sports related injuries, and Cardiac Death sign off's must be completed on Genesis. These are required once a year.

Students who have not completed the required forms by 10/30 WILL NOT BE ABLE TO TRY-OUT

• Physicals and Health History Update forms submitted to Genesis are then sent to our district Doctor to be approved for try-outs. The nurse does not approve any paperwork

• Students are NOT guaranteed a spot on the teams you are expressing interest in.

• If your child’s doctor prescribes albuterol, a completed Asthma Treatment Plan from the doctor is required before the school physician can approve your child’s sports physical.
• If your child’s doctor documents a food allergy requiring an EpiPen, a completed Anaphylaxis Treatment Plan from the doctor is required before the school physician can approve your child’s sports physical.
