Student registrations can be picked up and returned at the Hermosa school office or the Hermosa School main entrance vestibule from 7AM-4PM Monday-Friday.

Monument Health is proud to offer free sports physicals for student-athletes across our markets. These physicals are a convenient way to ensure young athletes are healthy and ready for the upcoming season, meeting all state and school activity requirements. Check the list below for dates, times and locations near you. Physical forms can be picked up at Hermosa School in the main entrance vestibule open Monday-Friday 7AM-4PM.
