October 1st and 2nd Vision Screenings for all kindergarteners,1st, 3rd and 5th graders will be
conducted during school hours. Students that wear glasses or contacts should be sure to
bring/wear them on the screening days. State law requires parents/guardians to be informed
that school vision screening is not a substitute for a complete eye exam/vision evaluation by an
eye doctor. School screenings assess students' visual acuity and do not evaluate eye health or
prescribe treatments. Parents/guardians will receive a referral letter if your student fails the
screening.
Parents/guardians who do not want their student to be screened at school must complete and
turn in an opt-out form. The Vision Opt-Out Form can be found here on the PHCS website in the
Miscellaneous section of Health Forms.
