Consent form for students in more than one sport
THIS IS ONLY FOR STUDENTS WHO HAVE ALREADY PLAYED ONE SPORT IN CONWAY AND HAVE ALL OF THE REGULAR SPORTS FORMS FILLED OUT
The informed consent form MUST be FILLED out for each sport a student plays.

You ONLY need to fill these forms out IF SOMETHING HAS CHANGED.
Emergency Card (this requires an actual signature)
Physical Certification: These are good for two years from the date of the physical.
Transportation Permission: Letting us know who your athlete can go home with.
You have three options with our forms:
1. Use the links below to fill in Google Forms and turn them in electronically OR
2. Print off the forms at the end of this document, fill out and turn them in OR
3. Have your student pick up forms in the office, fill them out at home and bring them back
