Ben Franklin Elementary School is pleased to welcome Mrs. Cynthia Brennan as our new school nurse. Mrs. Brennan graduated from Thomas Edison State University and The College of New Jersey and has spent the last 5 years working with students in schools. The last two years, Mrs. Brennan was the school nurse in Edison for a K-5 elementary school of over 600 students. When not helping 600+ students, Mrs. Brennan enjoys spending time with her own two girls, 12 and 15 years old.

"I hope to get to know all of you and your children in the next year. Don’t ever hesitate to reach out to me about any questions or concerns. I am here to ensure that the students at BF have a safe, healthy and happy school year! I hope you all enjoy the rest of your summer."

Below are the required forms that need to be completed before your child starts school.

PHYSICAL EXAM

All new students are required to have documentation of a recent physical exam from the child’s physician that was conducted within 365 days prior to the first day of school. It is suggested that returning students provide an updated physical exam to keep the Health Office informed of any changes. Click here for the form you will need.

IMMUNIZATIONS

There are immunizations that are required to attend school. These requirements differ based on age/grade. Parents/guardians are required to submit a physician certified list of all immunizations received to date. Click here to see the immunization requirements.

MEDICATION ADMINISTERED DURING THE SCHOOL DAY

If a student will need medication given during school hours, specific forms must be filled out by the child’s physician and the parent/guardian. This includes daily medications, as needed medications, short term medication therapies, and over the counter medications. Click here for the form you will need to complete with your child's physician.

Please send in all forms as early as possible. You can drop them off at the main office of the school or mail them to the school at:

If dropping off at the school during the summer, please call ahead to make sure someone is here as we currently have construction going on in the main office.

Please send all forms on or before the first day of school.
