New Senate Bill 12 Parental Consent Requirement
In accordance with new legislation, every Sunnyvale ISD student is required to have a signed parental consent form on file by September 1 in order to provide access to vital school services, including health care from nurses, counseling support, and athletic training.

Required forms for each child in Parent Portal are due. Deadline was September 1.
Examples of Physical Health Services:
- basic first aid such as a bandage application
- support during illness or physical symptoms at school
- broken or fractured limbs
- nose bleeds
Examples of Mental Health Services:
- short term individual counseling from the school counselor
- whole class guidance/character lessons from the counselor
- check-ins and support from the counselor during emotional distress or crisis
