UMMS Vaccine Requirements

• All students entering 6th grade must have a physical examination.
• Download the physical form HERE and submit by December 15, 2024.
• PIAA forms are only required for participation in Athletics. Click here for more information about Middle School Athletic requirements.

Send the physical form to the school nurse via one of the following options:

• mail or drop off: Upper Moreland MS, 4000 Orangemans Road Hatboro, PA 19040
  

• All students entering 7th-grade must have a MCV & TDAP vaccine by the first day of school.
• The vaccine requirements are due by September 3, 2024, the first day of 7th grade, 2024-25 school year.
• Students will be excluded from school if immunizations are not received by September 10, 2024.

If you are unsure if your child had this vaccine, please contact your primary healthcare provider for the date to be electronically sent to the school nurse via one of the following options:

Any student requiring the administration of medication during the school day (including over the counter, inhalers, & prescribed medications) will need a Medication Permission Form, signed by their parent and physician. Download the Medication Permission Form HERE.

*Please remember that a required physician’s treatment plan from the prescribing doctor is needed for asthma & food allergies. We ask that you hand in these completed forms when you drop off epi pens and inhalers to the nurse's office.
