Student Medication Information 26-27
Hello Cheyenne parents!!!
I cannot believe we are close to beginning another school year! This email is for students who have specific medical conditions, or those that will require medication to be kept in the clinic at school. This includes ALL prescription and over the counter medications. Please disregard this email if it does not apply to your student(s). Please read through the entirety of this email and contact me directly with any questions you may have! I hope you will find this email informative, and easy to understand. I look forward to seeing you all soon!
ALL FORMS ARE ATTACHED BELOW!
*Please try to bring all medication and paperwork during walk about days, or before the first day of school, Thursday August 13th.
*Any medication needed at school must be brought to the school by a parent or guardian. Any medication brought to the school without proper documentation will not be accepted. Additionally, if a medication is brought to the school by a student without documentation, the medication will be placed in the office and a parent or designated adult must come pick up the medication during school hours. This is for safety and legal purposes to protect us and your student(s). All forms must be current and dated AFTER July 1st for the 26/27 school year. That being said, if you filled out forms last year, we require new forms this year. This includes all medication forms, plans, and doctors notes.
*Please note that self carry medications are allowed only for inhalers, epi pens, diabetes medication, and pancreatic enzymes. If your student will self carry medication, please supply us with a backup for the office.
Additional Requests for Students with Specific Conditions:

Students with seizures: please fill out a " seizure action plan form" and a "medication authorization form." If your student will require more than one medication, an administration form for each medication is required.
Students with type 1 or type 2 diabetes: please fill out a "diabetes medical management plan" form and a "medication authorization form". Additionally, please submit a copy of the student's current diabetes plan from the doctor.
Students with food allergies or alternative nutritional requests: please fill out a "nutritional health plan form" and submit a letter from the doctor stating the specific food allergy and alternative nutritional request so it can be sent to the cafeteria. There is also a "milk substitution form" for those requiring soy milk. Please note that this form MUST be signed by a doctor.
