5Essentials PARENT Survey
One of the ways we get feedback is through the Illinois 5Essentials Survey. Every year, parents are able to take the survey to let us know how we are doing as a school. Your feedback is important.
The survey should only take approximately 10 minutes to complete. Our goal is to get 40% of our parents/guardians to take the survey. As of February 26th we have 18% of parents who have completed the survey. Click here to take the survey. Please complete the survey by March 13th. Thank you for your time!
5Essentials STUDENT Survey - 4th-5th Grade Students
February 2026
Dear Parent of 4th-5th Grade Students,
We are writing to inform you about a student survey that will be administered to 4th & 5th grade students the week of March 9th, called the Illinois 5Essentials Survey. This survey is based on 20+ years of research at the University of Chicago. It asks students about their experiences in school, giving school leaders critical student input to help develop school improvement plans.
4th & 5th grade students will be required to log-in using their state student ID and birth date in order to complete the survey. Students’ identities will be completely confidential. Teachers or administrators will never see individual responses to survey questions and will only receive aggregated information, such as the total percentage of students who agree homework assignments help them learn the course material.
Please be aware that under the Protection of Pupil Rights Act. 20 U.S.C. Section 1232(c) (1) (A), you have the right to review a copy of the questions asked of your student(s). Survey questions can be found on the Illinois 5Essentials website, CLICK HERE to view all the questions students will be asked.
If you do not want your son or daughter to participate, please email Mrs. Johnston -sjohnston@sd13.org by Friday, March 6, 2025.
Thank you,
Stacy Johnston
