A note from the school nurses...
As we wrap up this year for our Seniors, we wanted to share some valuable information regarding vaccine records. CHS clinic will be including a copy of your student's vaccine record to be included in their diploma envelope. We suggest that you take this out and scan a copy for you and your student. The MCV4 (Meningitis) vaccine is required for student's entering college and many technical programs. This vaccine would have been received during your student's Junior or Senior year as a booster to the one they received in 7th grade.
In addition, your student's vaccine record is entered on the Texas vaccine registry (ImmTrac) until their 18th birthday. As of their 18th birthday the record is archived unless the Adult Consent Form is filled out by them and submitted. Please do not worry if you child already turned 18, their record will remain in archive for approximately 6 months after and can be retrieved by filling out the form. We have attached the needed form for you. Please follow the instructions and send directly to ImmTrac not CHS.
This form allows for your student's vaccine record to remain on ImmTrac for their lifetime so that they have access to it.
If you have any questions please feel free to call the clinic at 830-221-2429 or email with questions. Thank you for trusting us with your children and best wishes for a bright future!
