All students who participate in 6th-12th grade athletics and 8th-12th grade band are required to have an annual physical on file before they are allowed to participate each school year. This includes practices. Physical must be filled out on the OSSAA Physical Examination and Parental Consent form. CLICK HERE to download and print the form. Please turn in all physical forms to your respective site office.

Mayes County Trail Alliance was awarded a grant to fund a comprehensive Safety Action Plan that will include a Safe Routes to School Plan, Bike / Walk Plan and a Safety Action Plan. Responses to the survey below will help to inform these plans by identifying current issues and concerns that should be addressed. The survey will be open until September 13, 2024. We appreciate your participation!
