RESIDENCY VERIFICATION PARTNERSHIP

Zion-Benton Township High School District 126 is excited to announce our partnership with CLEAR Residency Verification, a move designed to enhance efficiency and provide greater convenience for our families. This collaboration benefits the Zion-Benton community in several impactful ways:

  1. Reduced Administrative Burden: Families with stable residency will appreciate the elimination of annual residency document submissions, freeing up their time and effort.
  2. Seamless Registration Experience: This partnership facilitates a faster and more user-friendly registration process for all, leading to a smoother start to the school year.
  3. Equitable Resource Allocation: By ensuring the accuracy of student residency, we can more effectively allocate our resources and services to the students truly within our district, upholding fairness and maximizing educational impact.

Important Residency Verification Information for Zion-Benton District 126

For the upcoming school year, Zion-Benton Township High School District 126 is committed to accurately verifying the residency of all students. This ensures we can best serve our community and allocate resources effectively.

If we haven't been able to confirm your residency automatically, or if your student is new to the district, or if your family has recently moved, you will need to provide proof of residency. We encourage all families to begin gathering the necessary documentation now to ensure a smooth and efficient Online-Onsite registration process.

Required Proof of Residency Documents

To confirm a student's eligibility to attend District 126, we require one (1) document showing proper name and address from each of the following categories for verification every school year:

  • Closing Statement for recent home purchases
  • Letter from manager and proof of last month’s payment (for rental properties)
  • Notarized letter from landlord in lieu of a formal lease (for month-to-month agreements)
  • Letter of Residence: This document is to be used when the person enrolling a student is living with a current District 126 resident (refer to 7:60-AP2, E2 for details).

Category II – Proof of Occupancy (One document required)

  • Most recent cable television and/or credit card bill (dated June or later)

IMPORTANT HEALTH INFORMATION IMPORTANT HEALTH INFORMATION

Immunizations

Illinois School Code requires that all incoming ninth graders have a physical examination prior to their freshman year of high school. Students are required to provide proof of immunization for:

  • Tetanus/Diphtheria/Pertussis (Dtap, DTP, TdaP)
  • Polio • Hepatitis B • Measles/Mumps/Rubella (MMR)
  • Varicella (unless proof of having disease)
  • Meningococcal (MCV4)

All Illinois immunization requirements can be accessed at: https://dph.illinois.gov/content/dam/soi/en/web/idph/publications/idph/topics-and-services/prevention-wellness/immunization/immunization-requirements-chart-2021.pdf

The student’s original freshman physicals are kept on file in the Health Office and can be used for athletic participation. The original freshman physical examination must be completed on the Illinois Department of Human Services’ Certificate of Child Health Examination form, turned in during Onsite Registration or your student will not be allowed to begin the school year. You can obtain a copy of this form below.

A sports physical form does NOT qualify as a freshman physical and will NOT be accepted as such. If your student is participating in high school athletics prior to you completing Online Registration, please be sure to provide the Athletic Office with a copy of your student’s physical examination.

Please note the following on your student’s State of Illinois Physical Examination form:

  1. Health history must be completed, signed and dated by the parent or guardian.
  2. Immunization section must be completed, signed and dated by a doctor or the Health Department.
  3. The physical section must be completed, signed, and dated by a doctor.
