What is the NYC Public Schools Family & Student Survey?

Every year, we ask all families, teachers, and staff of grades 3-K through 12, and students in grades 6-12 to take the NYC School Survey. The survey gives school leaders a better understanding of what members of their school community think about their school. The information collected by the survey is designed to support a dialogue among all members of the school community about how to make the school a better place to learn.

The survey is about 40 questions and take around 10 minutes to complete. Survey results help parents and families get a better idea of the school environment, area of strengths, and areas that need improvement.

The survey also helps schools get additional funding and resources for programming, staff development and so much more.

Instructions for Families:

  1. If your student is in a NYCPS or Charter school, you will need your child’s nine-digit student identification number, or OSIS, to begin filling out your survey online.
  2. You can find the OSIS on your student’s report card or school ID card or on the paper version of the survey. Contact your child's school if you have trouble finding your student's OSIS.
  3. Once on the survey page, you must enter “f” and then your student's OSIS to access the survey. The “f” must be lowercase.
  4. If your child is in a Birth-to-Five program, please contact the school or program to receive your survey access code.

Note: Families can also access the survey by logging into their NYC Schools Account
