Annual Forms & Information Updates

All parents and caregivers are strongly encouraged to complete the Annual Forms & Information Updates online as soon as possible.

Paper forms previously sent home by schools will no longer be accepted. If you are unable to complete the online form, please contact your student's principal for assistance.

These forms include critical information such as the Device & Acceptable Use of Technology Agreements, student handbook acknowledgments, emergency contacts, permissions, special dietary needs, medical details, and IEP information. Submitting these updates ensures schools can provide safe, personalized, and well-informed support for every student.

Even if nothing has changed since last year, completing the forms is still required for every student.

If you prefer your child not be included in school photos or videos, you must indicate this by completing the Annual Forms & Information Updates online. This is the only way to officially opt out and ensure your preferences are recorded for the school year.

The Device & Acceptable Use of Technology Agreements are mandatory for all BCS students who use school-issued technology. Without a completed agreement on file, your student may not be allowed to access school technology.

Keeping contact and emergency information current is vital for your student’s safety and well-being. As part of the Annual Forms & Information Updates, families are required to review and update all emergency contacts, phone numbers, and addresses.

The primary and secondary parent/caregiver listed on the Annual Forms & Information Updates are automatically signed up to receive NTouch alerts for attendance and important updates from the district and your student's school.

Parents and caregivers are asked to review and update any medical and IEP information for their student as a part of the Annual Forms & Information Updates online.

Accurate medical details—such as allergies, medications, health conditions, and special diets—ensure that school staff can respond appropriately in case of an emergency. Likewise, up-to-date IEP information helps schools provide the right support and accommodations from day one.
