Dear JC 2nd and 3rd Grade Families,

Please check your email for a very important message regarding State of CT mandated Physicals. The State requires a physical dated from January of 2nd grade year through December of 3rd grade year to meet this requirement. Please send me an updated physical for your child health record with updated immunizations as soon as possible. I have attached the form you will need for this in my email, but please let me know if you need a hard copy, I will happily send one home with your child.

If you have already sent these in, please disregard this email. Specific dates are included in the attached memo so please read it in its entirety. If you are unsure if you have sent a copy, I would rather have an additional record to ensure we have met the state requirements.

Lastly, if your child has been vaccinated for COVID and has completed both vaccinations, please send me a copy of their card so I can add this information to their health record.
