Opt of our 10/30 Dental Screenings Here

Dental Screening

Our dental screening with Dr. Gordon will be held on October 30th.

If you would like to opt your child out of the dental screening, please click on this link: Dental Opt-Out Form

If you have already turned in paperwork showing that your child has been seen by a dentist this year, your child will automatically be opted out of the screening since they have already received dental care this year.

West Bay Smiles

West Bay Smiles is a dental service separate from the school dental examination. This program is designed to bring dental care directly to students who may be in need of dental services.

Services are provided during the school day, so you do not need to make a separate appointment or take your child out of school. If West Bay Smiles determines that your child needs to be seen at their dental office, they will contact you and discuss the next steps.

There is no dental insurance required, and there are no out-of-pocket costs for your child to receive services through this program.

If your child is in need of dental care and you would like them to participate in the West Bay Smiles program, please fill out this form or return the completed paper form to the school nurse.
