We are asking that you please complete and submit a meal application as soon as possible to ensure that additional funding for our school is available to meet the needs of our students. All information on the application submitted is confidential. Without your assistance in completing and returning the attached application, our school cannot maximize the use of available State and Federal funds.

SEPT.19: VISION SCREENING FOR YOUNGKINDER ONLY

Due to time constraints, we are unable to add students to the screening schedule outside of their assigned grade level. .You may however schedule an appointment at one of the OCHD clinics. Holland 616.396.5266

Parents may opt their child out of screening, written documentation is required and must include:

o Student's name, date of birth, teacher, and grade

o Which screening is being declined (hearing or vision) and screening date

o Parent's name and date

Please send written opt out of screening to the school office no later than Tuesday, September 16.
