Monthly Personal Care Activity Log Attached
Print Attached Personal Care Activity Log to Track and Bill for Daily Services
The 2026-27 Monthly Personal Care Activity Log has been updated to reflect the dates for this school year, as well as the Common Calendar holiday dates for Wayne County.
Download and print as needed to track the services you are providing for students on your caseload. You will then use your Monthly Activity Log to bill Medicaid in MISTAR Service Tracker at the end of each month. Be sure to verify the services you are going to log with a member of the student's Special Education team, as you can only bill for the *service(s) listed on the Authorization for Medicaid Services form.
*You will track and bill for one service per student per day of each month.
How to Fill in Your Monthly Personal Care Activity Log
When filling in your Monthly Activity Log, you must include all of the students on your caseload, regardless of their Medicaid eligibility.
1. Write the name of each student you are caring for in the Student Name column:
2. Enter a billable or non-billable service code for each student on your Activity Log.
