HEALTH & WELLNESS
PEMBROKE PINES K - 8
Before entering or attending school in Florida (Kindergarten through 12th grade), each student must provide a Florida Certification of Immunization (DH 680 form) documenting all required vaccinations.
Please review the attached document carefully to determine the immunization requirements for your child for the 2026–2027 school year. We strongly encourage families not to wait until the last minute to obtain the necessary documentation.
For additional information about childhood immunizations, please visit: www.immunizeflorida.org
You may also refer to the Immunization Guidelines and the Recommended Child and Adolescent Immunization Schedule for complete details regarding school requirements.
We appreciate your cooperation and support in helping
us maintain a safe and healthy school environment.
- Parents/Guardian should send any updated physical and immunization records from the most recent pediatrician visit to clinic-ppk8@franklin-academy.org
- If your child has a medical condition or an allergy, please contact Clinic at clinic-ppk8@franklin-academy.org to discuss their best health plan
If your child requires medication during school hours, please fill out the appropriate authorization form(s)
Students may receive medication at school only with written authorization from both a parent or guardian and a licensed healthcare provider. This requirement applies to all medications, including both prescription and over-the-counter (OTC) medications.
A Medication Authorization Form must be completed every 12 months, or sooner if there are any changes to the medication order.
Parents or guardians are responsible for:
- Completing their portion of the form
- Obtaining the healthcare provider’s signed authorization
The provider’s order must include:
- Student’s name
- Diagnosis
- Allergies
- Medication name and strength
- Dosage
- Time and route of administration
- Possible side effects
- Provider’s signature and date
Thank you for your support in helping us ensure the safe administration of medications at school.
