Important Reminders and Updates
11th Grade Immunization Records Due October 2
Attention 11th Grade Parents and Guardians:
All 11th-grade students must submit an updated immunization record to our school nurse, Mrs. Casondra Petty, by Friday, October 2, 2026.
The updated record must show proof of the required meningococcal booster vaccine (MCV4), unless the student received their first MCV4 dose on or after their 16th birthday.
How to Submit:
Immunization records may be submitted directly to Nurse Petty in the AMHS School Clinic or emailed to Casondra_L_Petty@dekalbschoolsga.org.
Immunization certificates must be properly completed and signed by a licensed Georgia physician or public health official. Religious exemptions require a completed Form 2208, and medical exemptions must be documented on Form 3231 and reviewed annually.
Please do not wait until the deadline to submit your scholar’s updated record. Thank you for helping us keep our RAM students healthy, safe, and in compliance with Georgia immunization requirements.

Student Community Service Hours & Documentation
As students complete community service activities throughout the school year, please remind them to properly document their community service hours using the Community Service Log attached to this newsletter.
Students are responsible for completing 10 community service hours outside of EIC. Participation in EIC will account for the other 10 hours of the 20-hour requirement.
Students should record their service activities on one Community Service Log and obtain the appropriate verification for all hours completed. Once all 10 community service hours have been completed, students should submit one completed Community Service Log to the AMHS Front Desk.
📌 Deadline: All Community Service Logs must be submitted by Friday, April 30, 2027.
Please encourage students to keep a copy of their completed log for their personal records.
Thank you for helping our scholars stay on track with their community service requirements!
