YRBS - Youth Risk Behavior Survey

Dear Berea-Midpark High School Parents/Guardians,

We are writing to inform you about an important opportunity for our students to participate in a nationwide survey that helps us improve school health and wellness programs: the Youth Risk Behavior Survey (YRBS).

What is the YRBS?

The YRBS is a voluntary, anonymous, and confidential survey developed by the Centers for Disease Control and Prevention (CDC). Please see the attached YRBS Fact Sheet.

The YRBS helps identify students’ health and safety habits so communities and schools can focus their efforts on making improvements where needed. Healthy lifestyles for students mean longer, more productive lives for young people, as well as improved learning in the classroom.

The survey will ask questions about:

  • Nutrition and physical activities.

  • Injuries and weapons.

  • Tobacco, alcohol, and other drug use.

  • Sexual health behaviors.

The results provide valuable, aggregated data that Berea Midpark High School and the district use to develop and enhance programs that better support the health, safety, and academic success of all our students.

Survey Logistics and Participation

The YRBS will be administered during school hours on Monday, November 24th, during 2nd period at Berea Midpark High School.

Your student's participation is completely voluntary and requires your written permission (passive or active consent/assent form). All responses are kept anonymous and confidential. No identifying information is collected, and students may skip any question they are uncomfortable answering.

The consent form is attached in the link below and a paper copy will also be sent home with your child. Please review this information carefully.

YRBS Opt In/Opt Out Consent Form - Electronic Form - https://forms.gle/ubnne1r2PH3uPsrV6

YRBS Opt-In/Opt-Out Form - Paper Form - Print and return to the main office.

Incentive for Returning the Form

To encourage a high rate of parental response and student participation, every Berea Midpark High School student who returns the YRBS consent form—regardless of whether you choose for them to participate or not—will receive a Dunkin' Donut Gift Card.

We strongly encourage you to review and return the form promptly to ensure our data accurately reflects the needs of our student body and to make sure your student receives their gift card.

If you have any questions regarding the YRBS and/ or would like to review the questions, please do not hesitate to contact Meredith Ondak, Coordinator of Behavioral Health and Wellness at mondak@bereaschools.org or 216-898-8300.

Thank you for your cooperation and commitment to the health and well-being of our students.

Sincerely,

The Berea Midpark High School Administration

PSAT Scores

PSAT Scores Now Available via CollegeBoard

Scores from the October administration of the PSAT 8/9 and PSAT 10/NMSQT are now available for students to view through their College Board student account or via the College Board’s “BigFuture School” app.

Each student receives a detailed score report that provides valuable insights into academic strengths and areas for growth across the tested skill areas. These reports are an excellent resource for students, parents, and teachers to identify learning priorities, plan next steps, and prepare for future standardized assessments such as the SAT.

We encourage all students and families to review the results together and use the feedback as a guide for continued academic growth. If you have difficulty accessing scores or have questions about your student’s report, please reach out to your school counselor for assistance.

Parent Teacher Conferences Parent Teacher Conferences

BMHS will hold Parent Teacher Conferences on Thursday, November 13, 2025, from 3:15 to 7:00 P.M. All parents are encouraged to attend and discuss your student's progress. Staff locator sheets and a school map will be available on the night of the conferences. To schedule a conference CLICK HERE.

Conferences will be held in person, unless, you click "ADD SPECIAL REQUEST" for a virtual conference. You must provide contact information for a phone or video conference for the teacher to contact you.

Check out this lovely note from Audreys Outreach about our Girls Soccer Team.

We had a heartfelt visit from the Berea Midpark High School Lady Titans Soccer Team last week. These young ladies raised money to support their local community Food Pantry! These young ladies girls pictured put in a lot of effort for this fundraiser so they won the chance to come visit Audrey’s and get a tour. We think some of them will even come back to volunteer! Thanks Carla, Jordyn, Molly, Kylee, Callie, Brianna, Brooklynne, Coach Nick and organizer Janelle (not pictured)! We look forward to seeing you again.
