NURSES NOTES
25/26 Student Action Plans
A new Action Plan for your student’s health condition must be submitted at the beginning of every year. Actions Plans for the next year must be dated after June 1st for the following school year.
Necessary paperwork can be found at https://www.leanderisd.org/healthservices/
Please note: If your student will participate in an extracurricular activity (i.e. Sports, Band, Dance, Cheer, etc) please have your physician fill out a self carry form so that we can ensure the appropriate emergency medication is with the student at all times. The only authorized medications that are allowed to be carried are Insulin, Emergency Seizure Meds, EpiPens, and Inhalers. No medications will be administered by the LISD staff without proper documentation.
If you have any questions or need additional paperwork please don’t hesitate to reach out. If your student’s medical condition has changed, please send me an email so I can update your student’s file.
Required Seventh Grade Immunizations for the 2025-2026 School Year
Get your incoming seventh grader vaccinated early before the busy back-to-school rush. State law requires one dose of the meningococcal conjugate (MCV4) vaccine and one dose of the tetanus-diphtheria-acellular pertussis (Tdap) vaccine for all students attending 7th grade. Keeping children vaccinated protects the health of both the children and the community. Without proper documentation of the required vaccinations or a valid medical or conscientious exemption, students are not allowed to attend school. Full vaccine requirements for seventh grade students are available for viewing or downloading on the Department of State Health Services (DSHS) Immunization Section website at https://dshs.texas.gov/immunizations/school.
If you have any questions about vaccines, please consult your healthcare provider or local health department. To learn more visit the DSHS Immunization Section website at www.ImmunizeTexas.com, call 800-252-9152, or email Immunization.Info@dshs.texas.gov.
Thank you for keeping your child immunized and free from vaccine-preventable diseases.
