Print Attached Personal Care Activity Log to Track and Bill for Daily Services

The 2025-26 Monthly Personal Care Activity Log has been updated to reflect the dates for this school year, as well as the Common Calendar holiday dates for Wayne County.

Download and print as needed to track the services you are providing for students on your caseload. You will then use your Monthly Activity Log to bill Medicaid at the end of each month. Be sure to verify the services you are going to log with a member of the student's Special Education team, as you can only bill for the *service(s) listed on the Authorization for Medicaid Services form.

*You will track and bill for one service per student per day of each month.
