Change in Learning Model Form Due January 29, 2021

If you would like to make a change to your student's current learning model for Term 3, you must fill out this form by Friday, January 29th. The form must be completed by a parent or guardian to confirm the change.

Do NOT fill out this form if you would like to keep your student's current learning model the same. Only fill out the form if you are changing your student's learning model.

This update to your student's learning model is for February 8-April 15, 2021. There will be an additional opportunity to change your student's learning model prior to the end of Term 3 for Term 4.

As a reminder, all families may choose to have their student learn remotely for the remainder of the year. If your student is currently remote and you would like to keep it that way, no action is necessary. If you would like your student to switch to remote learning, please indicate this on the form. Students choosing to remain remote, will be remote until at least April 15th.

The form will be closed by 3:05pm on Friday, January 29, 2021, so please submit it before then.
