D128 Continuous Improvement Survey
Feb. 15, 2024

Greetings DARING Community!
It is time once again for our district to join the Illinois State Board of Education (ISBE) in the statewide survey of school climate and learning conditions. The survey provides a unique opportunity for students, parents/guardians, and teachers to have a voice in improving our schools.
The Illinois 5Essentials Survey provides a comprehensive picture of a school’s organizational culture in an individualized report measuring five “essentials” critical for school success:
Effective Leaders
Collaborative Teachers
Involved Families
Supportive Environment
Ambitious Instruction
Prior research in more than 400 schools has shown that schools that were strong in at least three of these “5Essentials” indicators were 10 times more likely to improve student learning gains in math and reading than those that were not.
The 5Essentials Survey is incredibly important to all schools, state-wide, especially in that it gives the opportunity for students, parents/guardians, and teachers to make their voices heard. Please read below for details about each version of the survey.
Student Survey
Students will take the 5Essentials survey on Wednesday, March 13, during 2nd period, on a modified bell schedule. On the survey day, students will be provided with all the information needed to access their survey, and student responses will remain completely confidential.
The student version of the 5Essentials survey is extremely important. Not only is it an opportunity for our students to make their voices heard, but the student participation rate is also one factor that goes into each school’s summative designation score, which is reported each year on the school state report cards.
Because of the importance of this survey, we are aiming for 100% student participation and will be holding survey make-up sessions and/or asking students to complete the survey at home if they are absent on March 13.
Of course, at D128, we never require students to participate in surveys, even though we do want to hear all of their voices. 5Essentials is no different, and you, as the parent/guardian have the option of opting your student out if you choose to do so. Students who are opted out will not receive a link to the survey and will be asked to sit quietly while the class completes the survey on March 13. If you would like to opt out your student please do so by clicking on this link by Friday, March 1. If you would like to preview the 5Essentials Survey questions, you may do so by clicking on this link.
Parent/Guardian Survey
You - our parents and guardians - also have the opportunity to make your voices heard regarding our school climate and learning conditions. The parent survey will amplify your voice on the essential elements needed to continue to cultivate an academic community that develops graduates who are DARING. Furthermore, it will help us to understand the conditions at your student’s school and guide improvement.
To complete the 5Essentials survey, please click on this link. Please submit all surveys by March 29, 2024.The survey will take approximately 10 minutes. In order for a report to be generated based on parent/guardian responses, we need at least 20% of our parents/guardians to participate. Of course, we want much more than that, and we hope you will make your voice heard by responding to the 2024 5Essentials survey.
Please note: If a parent has more than one (1) child and they attend the same school, that parent may elect to participate multiple times. For parents with students enrolled in multiple schools, we recommend that you participate in the survey for each school. Only one parent per household may submit a survey for a given child. Your identity and survey responses are completely confidential and will never be connected to you or your child.
Teacher Survey
The 5Essentials survey is also administered to certified teachers whose primary responsibility involves teaching students for most of the school day and year. Our D128 teaching staff will be given the opportunity to participate in this very important survey during their February 19, 2024, Institute Day and will have until the end of the survey window on March 29, 2024, to complete their responses if they do not finish that day.
On behalf of the Illinois State Board of Education, UChicago Impact, the provider of the statewide survey, and our D128 community and administration, I thank you for taking the time to share information that will drive our actions toward continuous improvement.
