Winter Sports Information 2025-26

Varsity, JV & Modified sports may begin on November 17, 2025.

You will need a valid UBMD Sports Physical, as well as a completed FinalForms account in order to practice on the first day. Please see below for more information.

All athletes & parents will need to login to their FinalForms account to register for their desired Winter sport and complete the required seasonal documents.

FinalForms must be completed by parent and student prior to November 17th start date.

• The only sports season open for registration will be WINTER 2025-26.
• 7/8th graders will not be able to select a JV/Varsity Level sport without clearance from the Athletic Office. Please select the MODIFIED level of your sport until you pass the APP test. Once you pass the APP test, you will be manually added to the JV/Varsity level of your choosing by the Athletic Office. If you have existing valid APP test scores, please email Ms. Schaffert to add you to the JV/Varsity sport you would like.

Lockport Athletics transitioned from SportsWare to FinalForms last spring. If you received a sports physical last spring when they were offered during Phys. Ed. classes, please login to your Final Forms account to register for your desired Winter sport and complete the required seasonal documents.

If you are in need of a sports physical, please email Ms. Shaffert at sschaffert@lockportschools.net to set up an appointment on November 14th at LHS.

FinalForms must be completed by parent and student prior to November 14th. You will not be permitted into your UBMD Sports Physical if your Final Forms account is incomplete.

Helpful Notes:

• Parents must begin the registration process by creating their accounts, and inputting their child's information (parents of multiple athletes will be prompted to add additional children if necessary). Once the parent completes their registration and signs their portion of the forms, the student will then receive an email from FinalForms that their portion of the account is ready to be completed. (If the student uses a Lockport Schools email address for the account, please be sure to check ALL folders for the email. It may appear in your "Updates" folder instead of your inbox, etc.)

IF YOU ARE A 7TH OR 8TH GRADER LOOKING TO TRY OUT FOR A JV/VARSITY SPORT:

We are proud to say that we offer a Modified Sports Program for 7th & 8th grade students. Modified is the proper sports level for this age group. If your son or daughter wishes to try out for a sport at the high school level (Varsity or Junior Varsity), they would need to pass all aspects of the APP Test. Please note: Bowling & Golf do not require passing of the APP Test.

The APP test will be offered at Lockport High School on Friday, November 14th at 3:30pm by appointment only. Please contact Ms. Schaffert by email at sschaffert@lockportschools.net or by phone at 716-478-4474 to set up an appointment. Please enter through the Athletic entrance (Door 7), and please send your child with the required Parent Permission Form.

**Student-athletes can take the Sports Transfer bus from AMJHS to LHS immediately after school lets out on 11/14 (Bus A)- just be sure to send them with their signed Parent Permission Form, or they will not be able to take the test.
