A big thank-you to our wonderful school nurses for all that you do to care for and support our students every day! From helping when students aren’t feeling their best to providing comfort, care, and a reassuring smile, we truly appreciate everything you do for our school community.

We appreciate the families who have already submitted their child’s required health forms.

It is crucial that all necessary forms are provided to avoid any interruption in your child’s education as the State of Illinois REQUIRED EXCLUSION date of October 15th.

Our school nurses will be reaching out to families who need to submit updated information, so please be attentive to any communication via voicemail, email, or backpack mail. Physical/Immunization Record:

• Required for all kindergarten and new students

• The physical must be completed within a year of school start, dated on or after August 20, 2025

• Parents must complete and sign the health history section

• Immunizations Requirements must be met or include a MD approved schedule of administration

• Failure to submit the form and have it reviewed by the School Nurse prior to the OCTOBER 15th deadline will result in your child being EXCLUDED from school

Vision Exam Form:

• Required for all kindergarten and students new to the state of Illinois

• The exam must be completed by a licensed eye doctor and dated on or after August 21, 2025

• The official State of Illinois Eye Examination form is required

• Due date of October 15, 2026

Dental Exam Form:

• Required for all kindergarten, second grade and six grade students

• The exam must be completed by a licensed dentist and dated after November 15, 2025

• The official State of Illinois Dental Examination form is required

• This is due by May 15th, 2027

A friendly reminder to help ensure a smooth and safe drop-off process for all of our students each morning!

• Late Drop-Off (After 9:05 AM): If you arrive around 9:05 AM and the school doors are already closed, please park your car, walk your child into the main office, and sign them in using the Google Form.

• Reporting an Absence or Late Arrival: If your child will be arriving late or absent, please notify the office in advance using any of our attendance reporting methods. The easiest way is to call the school office, dial 1, and leave a voicemail message with your child's details.

A friendly reminder that our school dismissal time is 3:35 PM. Please plan to arrive on time for your child’s pick-up and follow all school pick-up procedures and safety guidelines.

Walker Parents: If your child is a walker, please be sure to arrive at the designated walker pick-up area at dismissal time and follow the staff’s directions to help ensure a safe and smooth dismissal for all students.

If you are unable to pick up your child at 3:35 PM, please consider enrolling in CAP services to ensure your child has appropriate supervision after dismissal.

What a fantastic evening at our Ivy Hill Fall Festival! Thank you to all our families, volunteers, and staff who came out to make the event such a wonderful success. From carnival games and inflatables to the glow party and delicious treats, it was amazing to see our school community coming together to celebrate and have fun!
