K-5 Student Intent Form 2026-27

To help us plan and staff for the next school year, we are asking parents to please inform us of your intentions regarding school placement for the upcoming school year.

Would you please take a few minutes to fill out the bottom portion of this form and return to the school by March 27, 2026. Parents/Guardians are also welcome to complete the form online.

You can also click on the attachment (below) and print out the form, fill it out and have your child bring it to the school.

Please ensure to fill out a form for each child that currently attends St. Kateri Tekakwitha School.

Select your child's teacher.

Step 4

Type your name.

Step 5

Select yes if your child will be attending St. Kateri School next year.

Select no if your child will not be attending St. Kateri School next year. If no, the school will contact you in regard to where your child will be going to school next year.
