FINAL FORMS – Remember, if you have any changes to your information (phone number, address, who can pick up your student, etc) throughout the school year, change it in Final Forms!. The office is notified of these changes and will update our information.
A MESSAGE FROM THE NURSE
TMS Parents, if you received a letter on September 5th stating that you needed to get your student's Tdap and Meningococcal immunizations completed, please remember that documentation needs to be completed by September 30, 2025. Exclusion starts October 1st, 2025.
We need one of the following:
- A medical vaccination record showing your student has received the vaccines.
- A phone call from a parent or guardian that an appointment has been made with the child's physician.
- A completed Certificate of Exemption form claiming you have a medical, personal, or religious reason for not vaccinating your student.
