HELLO!
Welcome back Watertown High School Students and Families! I hope you had a wonderful and relaxing summer. Please review information below about prescription and over-the-counter medication.
IMPORTANT INFORMATION
MEDICATION DROP OFF
For those students who will be receiving scheduled medications during the school day, a parent/guardian will need to bring in the medication along with this signed form here (also attached below). The medication also needs to be accompanied by a print out of the clinician's order for the prescribed medication. The medication itself must be in its original packaging and labeled with the student's name.
**Please note**: Students are not allowed to bring in the medications themselves unless they have written consent from their provider to self carry.
EMERGENCY MEDICATIONS
If your student has a life-threatening allergy requiring EpiPen use, asthma requiring inhaler use, seizure history requiring rescue medication, or diabetes, all medical information needs to be up to date in the Nurse's Office.
What Does This Mean?
Anaphylaxis Action Plan and EpiPen
Asthma Action Plan and Inhaler
Seizure Action Plan and Medication
Diabetic Care Plan and Supplies
Please come by the Nurse’s Office (8:15 am -3:05 pm M-F) to drop off emergency medications along with the corresponding action plans. The action plans must be up to date and not expired. Emergency medications need to be in the original packaging with the student's name on the package.
Students with Diabetic Care Plans should plan on bringing: Extra supplies (glucose and ketone strips), extra pods, needles, insulin to be stored in the nurse's office, and snacks that are consistent with those outlined in the care plan.
OVER-THE-COUNTER MEDICATIONS
During the year, your student might need an over-the-counter medication like Tylenol or Ibuprofen. Here is the consent form (also attached below) allowing these medications to be administered. Please read through and denote which medications are safe and acceptable for your student. Your student can return this form to the Nurse's Office.
THANK YOU!
Please do not hesitate to reach out to me with any questions or concerns!
