Letter from the Principal
At FMS, we have a goal to have 100% of our students connected to a trusted adult. To track this goal, we have 3 ways to check in with our students.
1. Fiver - This is a daily check-in during Trojan Time where students show a 1-5 rating on how they are feeling as they start their day, 1 being the lowest and 5 the highest. Students who are a 1 or 2 get a follow up with an adult in the building to get the support they need and ensure they are ready to take on the day.
2. Student Connection Survey - This is a survey we give to students up to 3 times per year, where we ask about how they perceive certain things at school and which adult, if any, they would go to at school for help.
3. Mental Health Screener - This is a screener we give to students up to 3 times per year, which provides indicators whether students are exhibiting warning signs of depression, anxiety, and suicidal ideation. The data from this screener provides our counselors with valuable information that our students may not otherwise share and allow counseling staff to intervene and/or refer students and families to needed services and resources.
Each of these resources require parents to opt in per state legislation. Many of you have done so during the registration process. However, many have not. If you're unsure or would like to change your response to opt your student in for one or all of these resources, please fill out the appropriate form below.
We greatly appreciate your support as we ensure a safe and caring environment for our students!

Strengthening Families Program
Strengthening Families Program for ages 10-14 starts just two weeks from tonight! Join school staff members, Jay Thompson and Bethany LaRose, along with Jefferson County Extension Staff, to help your family learn to manage emotions, set goals, avoid peer pressure, make better decisions, and create better relationships. Youth and their parents or caregivers will participate in fun, hands-on activities, games, and discussions to help parents and caregivers learn how to show love while setting limits, how to listen to their child, and how to help family members show appreciation to each other. All families have strengths; this program will help your family become even stronger!
This program will be offered on Mondays from October 13 to November 24 at the Fairfield Middle School at no cost to families. A meal is provided each week at 5:30 PM, with the program from 6-8 PM. To register online, go to: https://go.iastate.edu/BTMAAZ If you have questions, contact Jay Thompson at FMS at jay.thompson@fairfieldsfuture.org or Courtney Burkhalter at Extension at 641-472-4166 or cburk@iastate.edu.
