Important Message from Nurse Jenny:
Immunizations and Medication Guidelines for School Attendance
Hello Parents,
This is Nurse Jenny; I am the fulltime nurse at Valleyview Elementary. I hope you have all had a safe, healthy and relaxing summer!
As we prepare to start back, I wanted to remind you of a few things:
IMMUNIZATIONS
ALL KINDERGARTEN and NEW to DISTRICT STUDENTS
There are several vaccines required for school attendance and if you have not turned in an up-to-date Immunization Record, please do so as soon as possible.
Please do not to wait until the last minute to schedule an appointment with your child's healthcare provider.
Please bring to Valleyview, or you or your physician can email your child's vaccination records to jburley@columbus.k12.oh.us or fax a copy to: 614-365-5115
as soon as possible.
Immunization Exclusion Day is (Friday, September 23rd). If you have not turned in an updated Immunization record by this day, YOUR CHILD WILL NOT BE ABLE TO RETURN TO SCHOOL UNTIL an updated record is turned in to Nurse Jenny.
If you have questions about the vaccines needed to meet the State of Ohio school vaccination requirements, or if you need help finding a healthcare provider call Nurse Jenny 614-365-5099.
You can also contact your family physician or Columbus City Schools Health Services: 614-365-5824.

MEDICATION AT SCHOOL
If medication needs be taken during the school day please follow these guidelines:
Submit a completed, signed Medication Authorization Form to the School Nurse. The nurse can supply this form.
The parent/guardian must complete the bottom section of the form and a healthcare provider must complete the top section for the medication order.
A new Medication Authorization Form must be completed each school year or whenever there is a change in the medication or dose.
All medication must be in the original container in which it was dispensed by the prescribing physician, healthcare provider or licensed pharmacist.
School personnel cannot give students over-the-counter medication unless it has been prescribed by a healthcare provider and a Medication Authorization Form is completed. Prescribed over-the-counter medications follow the same guidelines as stated above for prescribed medications. (Over the counter medications include pain medication such as Tylenol, cough medicine, and ointments.)
Medications ordered three times a day or less, unless there is a time specified, may not need to be given at school.
The medication may be given before school, after school and at bedtime. The container must be labeled with the correct dose and instructions.
The label must match what is on the Medication Authorization Form.
Students taking a medication at both school and home can request 2 separate labeled bottles from the pharmacy to divide the pills for home and school.
MEDICAL CONDITIONS or CONCERNS
Just a reminder to update Nurse Jenny if your child has any medical conditions that may impact them at school. In the event of an emergency a thorough medical history can determine the steps taken to treat, manage or process the situation. The information will remain confidential unless your child's condition calls for an Emergency Action Plan. This is a plan that notifies your child's teachers and school staff members interacting with them of the steps to take in the event of an emergency, for example: diabetes, asthma, seizures, food allergies/anaphylaxis. IF you have ANY questions or concerns, stop by any time (my door is always open) or send an email and I will get back to you as soon as possible.
