Nurse's Notes
Spring 2025
Happy Spring Watertown Middle School Families!!!!
This time of year brings a lot of excitement with the sun shining and warmer weather! There is also a lot happening in the middle school with fieldtrips, completion of screenings, and end of the year events. Please read the information below. I am available to answer any questions or concerns via email or you can call the nurse's office.

- Washington D.C- 8th Graders- Mr. McDermott has sent out a packet with information regarding daily medication your student may need to take while away on the trip. Please fill out the forms accordingly. Each prescribed medication will need it's own sheet filled in by your student's clinician and signed by a parent/guardian. This also includes allergy medication (Zyrtec, Claritin etc.) as those are not part of the over the counter medication subset. If you would like your child to be able to receive over the counter medications such as Tylenol or Ibuprofen while on the trip, and they do not already have consent on file for this school year in the nurse's office, please fill out the OTC form attached below. The prescribed medication order is also attached below. These forms are due by Friday, May 2nd. The collection of medications deadline will be Friday May 23rd. Any prescribed medications will need to be brought in by a parent/guardian to the nurse, NOT the student. Medications must be in original pill bottle with student's name and medication information. Please only send the amount of pills needed for the trip. If your child already has an allergy action plan, asthma care plan, seizure action plan, or diabetic care plan on file no further documentation is needed.
- 7th Grade Screenings- Mandated screenings will continue through the end of the year. This also includes SBIRT and BMI screenings for those students who have not been Opted Out. As a friendly reminder, the SBIRT screening is a tool to assess for risk factors surrounding alcohol and drug use. If you would like your child opted out for these screenings and have not done so already, you can notify me in writing via email.
- Fieldtrips- Springtime means lots of Fun and Educational Fieldtrips! If your student has an Epi Pen for a life threatening allergy or an inhaler for Asthma, we must have action plans on file in the nurse's office for the student to attend these events. For the students that self carry their Epi Pens or Inhalers, they must be brought to the nurse's office the day before the fieldtrip. The inhalers/epi pens will be carried by a designated personnel and dispersed to students once arrived at the location.
Thank you!
