NOSD District Nurse Update

Students entering 6th grade often need an immunization shot, please check with your doctor to make sure you are up to date with immunizations. Below are the requirements and form.

Mail a new copy to the District Nurse to update your child’s record when any additional dose(s) of vaccines are received. If your child still needs any of these vaccines, we encourage you to have your child immunized well before school starts to avoid the late summer rush. Waivers are available for religious, health and personal conviction reasons.

Secure computer vaccine records may be available for parents to view through the Wisconsin Immunization Registry (WIR). http://dhfswir.org. For more information on WIR or obtaining your child’s records on WIR, please contact the Washington / Ozaukee County Public Health Department.
