Form 1095-C
Tax Information
Form 1095-C is an informational tax document detailing the health insurance coverage offered to you as an employee. This form is not required to submit for federal tax returns.
Beginning this year, CCPS will not be printing and mailing these documents to employees.
Employees may access their individualized form in their Benelogic File Cabinet. To access Benelogic follow the steps below:
- Using the Internet, go to https://ccps.benelogic.com/
- Enter your User ID which is your last name and the last four digits of your Social Security Number (example: smith1234).
- Enter your Password. If this is your first time logging into Benelogic, your password will be the last four digits of your Social Security Number (example: 1234). If you do not remember your Benelogic password, click on the link, “Forgot User ID/Password?”.
- Update your password information, if requested.
Employees will need to sign an ACA e-consent form and you will then be granted access to your 2025 1095-C form in your online File Cabinet.
