Mobile Immunization Appointments

High school students can receive required immunizations by appointment during the Free Sports Physical event at WVU Medicine Berkeley Medical Center on June 2 and 3. Appointments must be scheduled through the Berkeley County Health Department by calling 304-263-5131, Option 1 for Clinical.

Families will need to provide:

• Student immunization record
• Insurance information
• Student full name, date of birth, address, and gender
• Parent/guardian contact information
• Preferred appointment date (June 2 or June 3)

If the student is not already a Berkeley County Health Department patient, immunization records must be submitted prior to the appointment. Insurance information may be emailed, faxed, or dropped off in advance.

The deadline to request an appointment and submit all required information is 3 p.m. on Thursday, May 28.

Appointment times will be scheduled after all information is received. A parent or guardian must attend the appointment with the student.
