End of Year Reminder: End of Year Reminder:

Medication Pick-Up and Forms For 26-27 School Year

MEDICAL FORMS AND MEDICATION

Dear Parents,

It’s hard to believe there are only a few weeks of school left. This is a reminder that you will need to come to school to pick up your child's medication by 3pm on the last day of school which is Wednesday, May 27th. Medications that are not picked up by 3pm at the end of the school year will be disposed of according to the D303 policy. This policy also does not allow medication to remain in the health office over the summer for reuse in the fall. Medication must be picked up by a parent or adult designee with the parent’s permission. If a Medication Self Carry/Administration form was completed, meds will be sent home with your child in their backpacks. The Self Carry form is only for inhalers, Epi-Pens and topical creams. Any pills or liquids still need to be picked up by a parent.

Looking ahead to the 2026-2027 school year, if your child requires medication or any action plans, your child will need new physician orders and action plans for the upcoming school year. Ideally have the physician date the forms following the last day of school (5/27/26). Forms can be found below under section titled "Health Conditions & Care Plans" or under the health services page on the D303 website.

MEDICATION AUTHORIZATION

ASTHMA

LIFE THREATENING ALLERGIES

DIABETES

SEIZURE ACTION PLAN

All forms will need to be completed in full before any medications can be administered.

These forms can be submitted to the main office or emailed to the school nurse at bel-nurses@d303.org. Please reach out if you have any questions. The health office direct line is 331-228-2104.

Thank you!

When to keep your child home When to keep your child home

Consider keeping your child at home for an extra day of rest and observation if he or she has any of the following symptoms:

  • Very stuffy or runny nose and/or cough

  • Mild sore throat (no fever, no known exposure to strep)

  • Headache

  • Mild stomach ache

Definitely keep your child at home for treatment and observation if he or she has any of these symptoms:

  • Fever (greater than 100 degrees)

  • Vomiting (even once)

  • Diarrhea

  • Chills

  • General malaise or feelings of fatigue

  • Frequent congested or croupy (barky) cough

  • Lots of nasal congestion with frequent blowing of nose

To help prevent the flu and other colds, teach your children good hygiene habits:

● Wash hands frequently

● Do not touch eyes, nose or mouth

● Cover mouth and nose when sneezing or coughing, use a paper tissue, throw it away and then wash hands

● Avoid close contact with people who are ill

Colds are the most contagious during the first 48 hours. A child who has a fever should remain at home until "fever free" for a minimum of 24 hours without medication. A child who has started antibiotics needs to be on medication for 24 hours before being considered non-contagious and able to return to school. Keeping your ill child at home will minimize the spread of infections and viruses in the classroom.

Medication Medication

If your child requires medication during school hours, every effort should be made to administer the medication at home. However, if your Health Care Provider (HCP) determines that it is in your child’s best interest to take medication (either prescription or over-the-counter) during school hours, a D303 Medication Authorization Form must be completed and signed by the parent/guardian and the HCP and then submitted to the school nurse.

  • A Medication Authorization Form is required for each prescription or non-prescription medication, including cough drops, to be administered at school
  • Medication must be brought to school in its original container by a parent or responsible adult designee
  • Under no circumstances will the nurses administer any medication that is sent to school in baggies, envelopes, etc.
  • When controlled medications are dropped off at school, the medicine will be counted/measured by the nurse in the presence of the parent or designee and then signed off by both parties before being accepted
  • If necessary, the school nurse will contact the prescribing HCP to clarify the medication order or to consult with the HCP regarding the student’s reaction to the medication
  • A student may carry and self administer his/her own Quick-Relief Asthma Inhaler, Epinephrine Auto-Injector and/or Pancreatic Enzymes provided that a Self-Administration Parent Authorization form is on file in the school health office and all other requirements have been met
  • A parent or responsible adult designee must pick up any remaining medication by the last day of school. All medication not picked up at the end of the school year will be discarded
