Hello MPES Parents and Guardians,
We'd appreciate your participation in a brief survey surrounding ideas around the start up of a monthly parent group. Your feedback would help to determine a direction for the group as well as the best times and days to offer such a group. If you have questions regarding this survey please reach out to:

Notes from the Nurse:
The Iowa County Public Health Department will be visiting the Mineral Point School District on Tuesday October 29th to administer flu shots. There will only be flu shots no nasal mist.
The team will begin at the Elementary school at 9:00 am and remain on site for approximately two hours then they will transition to the Middle/High School for another two hours. These sessions are the only ones available at the school sites. You may go to the Iowa County Public Health Department in Dodgeville for free flu shots as well.
Please thoroughly read the attached documents, completely fill out and return to classroom teachers in the Elementary grades and Middle/High school aged students will hold onto the documents until the 29th of October.
Staff members may have a flu shot administered during these two sessions as well. A separate consent form for staff members will be available via email.
Please contact District School Nurse, Julie.pompos@mp.k12.wi.us if you have any questions.
Hard copies of all the forms will be accessible in the offices of each school. You can also find the form here.
