Prior Written Notice for a Reevaluation Form and Request for Consent for a Reevaluation Form
The revised and the annotated versionsare now available on the PaTTAN website at www.pattan.net. The Special Education Forms tab is located on the top of the home page for easy access to the revised and annotated forms. A training video on these forms is available at: https://www.pattan.net/Legal/Special-Education-and-Gifted-Forms. LEAs are required to start using these forms by November 1, 2023.
The Bureau of Special Education id working with OVR to get a driving simulator to every IU to support driving training for individuals with disabilities. Photo is an example, and may not be the actual simulator used.
