Dear Parents and Guardians,
The sports physical packets are available to all families. Please see your Primary Care Doctor (Pediatrician) for clearance to participate in sports. The new forms are on our website. The Doctor keeps the physical and the only papers returned to the school are the clearance form completed by the doctor and the individual sign off sheets that you have read the pamphlets on sudden cardiac death, concussion and opioids. Any physical turned in last school year that is still current, but is due to expire after the first day of practice for that individual sport, will be good until that sport season is completed. All physicals that are over 90 days old from their original date of completion will need to fill out a health history update, which is completed by the parent. The clearance forms still need to be reviewed by our School Doctor, so please get them in, in a timely manner.
