WINTER START DATES & COACHES CONTACT INFO
Varsity, JV & Modified sports may begin as early as November 16, 2026.
The sports offered, times & locations of tryouts, and contact information for coaches can be found here.
You will need a valid physical, as well as a completed FinalForms account in order to practice on the first day. Please see below for more information.
NEW PROCEDURES & POLICIES
Effective July 2026, the following new procedures and policies are in place for Lockport Athletics:
Sports Physicals- Sport physicals are no longer required to be through UBMD. Your regular Primary Care Physician physical will now be accepted, and should be uploaded directly to your Final Forms account for review by the school nurses. See directions on this process here. Sports physicals will no longer be offered through LCSD.
APP Testing- As of July 29, 2026, the New York State Education Department has made the following changes to the Advanced Placement Process (APP) for students in 7th and 8th grade who wish to try out for a Freshman, Junior Varsity (JV), or Varsity sport:
*Please note that these changes only apply to 7th & 8th grade students seeking to participate at the high school level. If your child will be participating in a modified sports program, these new requirements do not apply.
- The physical fitness test has been eliminated and is no longer part of the Advanced Placement Process.
- The Tanner Score recommendation has been eliminated.
- The school district must recommend a student-athlete for participation through the Advanced Placement Process.
The new process:
- Parent consent and a school invitation are required to participate in the Advanced Placement Process or to "play up"
- Any parents who believe their child may qualify to play up from the modified level/APP must complete the APP Parental Request for Invitation Form.
- After a parent completes the Request Form, the student is added to the JV/Varsity roster on Final Forms by the Athletic Office, and are cleared to tryout for that sport.
- Please note: Passing the APP test does not guarantee your child a spot on an upper level sports team, it merely allows them to participate in tryouts.
RETURNING ATHLETES
All athletes will need to login to their FinalForms account to register for their desired Winter sport and complete the required seasonal documents.
FinalForms must be completed by parent and student prior to the November 16th start date.
- The only sports season open for registration will be WINTER 2026-27.
- 7/8th graders will not be able to select a JV/Varsity Level sport without clearance from the Athletic Office. Please complete the APP Parental Request for Invitation Form. Once reviewed, you will be manually added to the JV/Varsity level of your choosing by the Athletic Office. Please note: this does not guarantee your child a spot on an upper level sports team, it merely allows them to participate in tryouts.
NEW ATHLETES
All athletes will need the following in order to participate in Lockport Athletics:
- A completed FinalForms account. If you received a sports physical last spring when they were offered during Phys. Ed. classes, please login to your Final Forms account to register for your desired Winter sport and complete the required seasonal documents.
- A valid physical uploaded to your Final Forms account. You may call your Primary Care Physician to get a copy of your most recently completed NYS Health Exam Form and upload it to your Final Forms account. Please see instructions on uploading here.
- To ensure timely review of physical paperwork on Final Forms, please adhere to the following upload deadlines:
All physicals must be uploaded by no later than November 11th for timely review by the school nurse.
FinalForms must be completed by parent and student prior to the November 16th start date. You will not be eligible to participate until fully cleared on Final Forms.
To set up your Final Forms account:
Click on the Parent Playbook below to get started in FinalForms. If you require any support during the process, scroll to the page bottom-right and click "Get Help" to reach the 24/7 support team.
Please click the link below and follow the prompts to create your account, create your students and sign your forms:
Helpful Notes:
- Parents must begin the registration process by creating their accounts, and inputting their child's information (parents of multiple athletes will be prompted to add additional children if necessary). Once the parent completes their registration and signs their portion of the forms, the student will then receive an email from FinalForms that their portion of the account is ready to be completed. (If the student uses a Lockport Schools email address for the account, please be sure to check ALL folders for the email. It may appear in your "Updates" folder instead of your inbox, etc.)
- The only sports season open for registration will be WINTER 2026-27.
- 7/8th graders will not be able to select a JV/Varsity Level sport without clearance from the Athletic Office. Please complete the APP Parental Request for Invitation Form and the Athletic Office will add you to the upper-level sport roster of your choosing.
APP TESTING
IF YOU ARE A 7TH OR 8TH GRADER LOOKING TO TRY OUT FOR A JV/VARSITY SPORT:
We are proud to say that we offer a Modified Sports Program for 7th & 8th grade students. Modified is the proper sports level for this age group. As of July 29, 2026, the New York State Education Department has made the following changes to the Advanced Placement Process (APP) for students in 7th and 8th grade who wish to try out for a Freshman, Junior Varsity (JV), or Varsity sport:

- The physical fitness test has been eliminated and is no longer part of the Advanced Placement Process.
- The Tanner Score recommendation has been eliminated.
- The school district must recommend a student-athlete for participation through the Advanced Placement Process.
The new process:
- Parent consent and a school invitation are required to participate in the Advanced Placement Process or to "play up"
- Beginning this season, any parents who believe their child may qualify to play up from the modified level/APP must complete the APP Parental Invitation Request Form.
- After a parent completes the Request Form, the student is added to the JV/Varsity roster on Final Forms by the Athletic Office, and are cleared to tryout for that sport.
*Important Note-
- 7/8th graders will not be able to select a JV/Varsity Level sport without clearance from the Athletic Office. Please complete the APP Invitation Request Form and the Athletic Office will add you to the upper-level sport roster of your choosing.
