Is Your Child Ready for School?

  • New Clothes?- Check
  • New Shoes?- Check
  • Backpack?- Check
  • Physical? Immunization? Medical Orders? Dental exam? Eye exam?- not done

Students entering Kindergarten, sixth, ninth and twelfth grade are required to have a physical exam and/or immunizations for school attendance.

Students who want to participate in sports at Cyd Lash Academy must have a sports physical, dated within the last year, on file in the health office.

Any student who needs feedings and medical procedures done at school must have new medical orders every Fall.

All students who require medications to be given during school must have new medical orders every Fall.

Make sure to make an appointment soon to get your student's requirements completed before school starts this Fall. All forms that your student needs have been sent home in their registration packet. Any questions, please contact your building nurse.

Check out the Health Department flyer for a list of resources.
