Parent Survey: Learning Options for 2nd Semester

Parents and Guardians,

If you want to change your student’s learning option placement (in-person, virtual, etc.) for 2nd semester, you can use the link below to submit your request. The form will open on Monday, November 30, and it will close at 5:00 pm on Friday, December 11.

You must be logged into Google using your student’s school email address in order to access the form. If you have multiple children for whom you are requesting a change in placement, please submit a different request for each child, but please only submit one form per student. If you change your mind after submitting a request, contact your student’s guidance counselor, and he or she will make the correction on the school’s side.

Please note that the in-person (i.e. traditional) learning option is subject to change depending on the levels of response needed to address changing public health conditions. The school may be in full in-person mode with face masks and other precautions, a staggered schedule mode where students alternate between in-person and remote learning on a special weekly schedule, or a full remote option where all students and employees are working from home. There is no guarantee that we will remain in any one of these modes for the rest of the school year.

**You only need to respond to the form link below if you want to CHANGE your student’s learning option for the 2nd semester:

BONUS QUESTION

We have heard from some families that they would like to consider continuing full-time virtual learning from home in 2021-22. We expect the COVID-19 crisis to be diminished by that time, but some families have become accustomed to the independence and flexibility of full-time virtual learning. In order to know whether to establish a permanent virtual school option, we would like to know how many families are interested. If this is an option that you would consider for one or more of your children, please use the link below to let us know of your interest.

This form will also be open from November 30 to December 11, 2020:
