Notice of and Consent for School Health-Related Services
Notice of and Consent for School Health-Related Services
Notice of and Consent for School Health-Related Services Health-Related Services Form
In accordance with law, the Magnolia ISD must provide parents with written notice of each school-based health-related service offered at the campus their child attends. These routine services promote student safety, wellness, and readiness to learn. School health-related services are most often provided by the school nurse. However, additional school staff may also provide first aid and care for ill or injured students. This would include, but is not limited to, teachers, athletic trainers, administrators, counselors, clinic assistants, and other staff charged with supervising children. This consent does not take the place of an individualized health plan, 504 plan, or other legally required document.
A parent has the right to withhold consent for or decline any health-related service.
Routine Health-Related Services Provided at Your Child’s Campus:
General First Aid
Checks of temperature, blood pressure, and pulse
Support during illness or physical symptoms at school
Listening to breathing or heart sounds
Examination of pupil responsiveness
Monitoring of chronic health conditions (e.g., asthma, diabetes)
Vision and hearing screenings
Lice screening
Coordination of health services
Heat illness prevention and injury support for student athletes
Any additional health-related services that the campus may believe are necessary and appropriate will necessitate separate written consent and parental involvement, unless during an emergency or as allowed by law.
Consent given through this form is effective during the current school year unless revoked earlier.
**A form should be filled out for each child**

Background Check Needed
We absolutely love our volunteers and know our school would not function without them! If you are planning on volunteering in any capacity or chaperoning on the field trips, get a head start by getting your MISD background check completed! (It would be better to have it done and not need it, then to need it and not have it!) We are so grateful for you!
