MySAEBRS or Social, Academic, Emotional, Behaviors Risk Screener.
ISD 728 is committed to supporting every student’s academic growth as well as their social, emotional, and behavioral growth. Part of our district’s Strategic Action Plan for ISD 728 students is for them to be able to regulate their emotions, develop positive relationships, and make responsible decisions. As part of this process, all students in our school will be completing a screener in the fall, winter, and spring called the MySAEBRS or Social, Academic, Emotional, Behaviors Risk Screener. The screener will ask students a total of 20 questions.
If you have questions about the screener or would like to request for your student not to participate, please contact Marco Voce, ZMHS Principal at 763-241-3505 X2801 or marco.voce@isd728.org or Rachel Hilyar, ISD 728 Director of Prevention & Safety at (763) 241-3400 x5003 or rachel.hilyar@isd728.org.

Updates from Health Services
Important Reminder for Families: Student Immunization Documentation
Minnesota law and district policy require that all students have up-to-date immunization records or a valid exemption on file in order to attend school. This ensures a safe and healthy learning environment for all
students. What does this mean for families?
● Families must provide documentation of their child’s immunizations or submit a valid exemption form
(medical exemption or conscientious objection).
● If your child is missing paperwork, they may not be allowed to attend school until the records are
received.
● Documentation can be submitted in the following ways:
○ Send a copy to your child’s school health office
○ Drop off paperwork at your school’s main office
Exemptions:
● Medical Exemption – signed by a healthcare provider.
● Conscientious Objection – notarized form signed by a parent/guardian.
Next Steps for Families:
✔ Review your child’s immunization records with your healthcare provider.
✔ Submit updated immunization paperwork or exemption forms to your school as soon as possible.
✔ Reach out to your school’s health office if you have questions about what is still needed.
Thank you for working with us to ensure that every child can remain in school ready to learn!
Respiratory Illness is Here
We are already seeing an increase in respiratory illnesses in our
school buildings. To help keep our students, staff, and families
healthy, please take a moment to review the latest information
and recommendations regarding respiratory illnesses from the
Minnesota Department of Health and the Centers for Disease
Control.
Respiratory illnesses affect the lungs and airways, causing symptoms such as:
● Upper respiratory symptoms: Runny or stuffy nose, sore throat, cough
● Systemic symptoms: Fever, chills, fatigue, headache, muscle aches
● Gastrointestinal symptoms: Vomiting or diarrhea
Prevention Tips
To protect yourself and others:
● Practice good hygiene: Wash hands regularly and clean commonly touched surfaces.
● Improve indoor air quality: Increase ventilation and consider using air purifiers.
● Use masks and practice physical distancing: Especially in crowded or poorly ventilated areas.
● Stay home when sick: Avoid contact with others until symptoms improve and you're fever-free for at
least 24 hours without medication.
Hearing & Vision Screenings
Please be advised that Health Services is no longer conducting mass hearing and vision screenings for students. However, if you have concerns about your child's hearing or vision, you may request a screening by contacting your building’s Licensed School Nurse or by filling out this request form.
